
Below is a list of commonly used financial aid forms and a description of their purpose. Please read the instructions on each form carefully and submit the form with the required documentation.
Financial Aid Forms
- California Dream Act Application
- This form is used to apply for financial aid by students who qualify for an AB 540 nonresident tuition exemption.
- Free Application for Federal Student Aid
- This form is to apply for financial aid by students who are citizens and eligible non-citizens.
- Ineligible PLUS Loan Borrower Form
- Parent(s) listed on the FAFSA can submit this form if they were unable to complete the online PLUS application because of their non-citizen status, a federal loan default, or a grant overpayment. Do not use this form if your credit was denied. Please have each parent listed on the FAFSA submit this form.
- Native American Opportunity Plan
- Graduate students – complete the Native American Opportunity Form to provide supporting documentation to verify eligibility for Native American Opportunity Plan.
- Undergraduate students can confirm eligibility the Native American Opportunity Plan by logging into the Berkeley Financial Aid Forms website. If the form has not yet been assigned to you, after logging in, click on the Manage Requests button at the top right, and click the + to open the form.

- Use this form if you are in need of historical information related to your financial aid.
