Below is a list of commonly used financial aid forms and a description of their purpose. Please read the instructions on each form carefully and submit the form with the required documentation.

2020-2021 Forms

2020-2021 Academic Year

California Dream Act Application 2020-21

Available October 1, 2019. This form is used to apply for financial aid for the 2020-21 academic year by students who qualify for an AB 540 nonresident tuition exemption.

Free Application for Federal Student Aid (FAFSA) 2020-21

Available October 1, 2019. This form is used to apply for financial aid for the 2020-21 academic year.  

Housing Security Deposit Application 

The Housing Security Deposit Award Fund was established to empower student success in the face of economic injustice. This self-generating award is funded by student fees and was created through a popularly supported student fee referendum. Students are now helping enrolled students who need assistance in paying security deposits to obtain safe, secure, and stable housing

Application for Additional Food Assistance  If you lack access to food in the short-term and are waiting for your CalFresh benefits, or you are not eligible for CalFresh benefits, applying to our food assistance program may be an option for you.

Income Verification Forms 

If you have been selected for verification, you can see what is needed by loging into the Berkeley Financial Aid Forms website. New to the Berkeley Finacial Aid Forms website? Check out Getting started in the Berkeley Financial Aid Forms Website for instructions and more information. 

Newly admitted students, you will be able to access this website and proceed with the verification process after you accept your offer of admission and create a CalNet ID

Ineligible PLUS Borrower Form 2020-21 Parent(s) listed on the FAFSA can submit this form if they were unable to complete the online PLUS application because of their non-citizen status, a federal loan default, or a grant overpayment.  Do not use this form if your credit was denied; see the options if credit is denied section (#8) on the PLUS Application Tips page. Please have each parent listed on the FAFSA submit this form.
Request for Confidential Information  This form is used to request copies of documents that contain confidential information. ​

Parent Contribution and Student Contribution Appeals - AVAILABLE MAY 1, 2020.

Has there been a change in your family's circumstances that is impacting your ability to contribute? You can submit a Parent Contribution Appeal or a Student Contribution Appeal - we will review your appeal, make adjustments (as appropriate) based on your most current financial situation. 

Newly admitted students, you will be able to access the website that allows you to submit an appeal after you accept your offer of admission and create a CalNet ID.

Childcare Expense Appeal You may submit this appeal if you would like to request an adjustment to your Summer Cost of Attendance for child care expenses.
Summer Intercampus Financial Aid Application This form is for students taking Summer courses through another UC campus. You must complete this application in order to receive financial aid for your Summer intercampus enrollment.
