Below is a list of commonly used financial aid forms and a description of their purpose. Please read the instructions on each form carefully and submit the form with the required documentation.

2019-2020 Forms

2019-20 Academic Year

California Dream Act Application 2019-20

Available October 1, 2018. This form is used to apply for financial aid for the 2019-20 academic year by students who qualify for an AB 540 nonresident tuition exemption.
Free Application for Federal Student Aid (FAFSA) 2019-20 Available October 1, 2018. This form is used to apply for financial aid for the 2019-20 academic year.  
Ineligible PLUS Borrower Form 2019-20 Parent(s) listed on the FAFSA can submit this form if they were unable to complete the online PLUS application because of their non-citizen status, a federal loan default, or a grant overpayment.  Do not use this form if your credit was denied; see the options if credit is denied section (#8) on the PLUS Application Tips page.  Please have each parent listed on the FAFSA submit this form.
Income Verification Forms

Newly admitted students, you will be able to access this website and proceed with the verification process after you accept your offer of admission and create a CalNet ID

Parent Contribution and Student Contribution Appeals

Has there been a change in your family's circumstances that is impacting your ability to contribute? You can submit a Parent Contribution Appeal or a Student Contribution Appeal - we will review your appeal, make adjustments (as appropriate) based on your most current financial situation. 

Newly admitted students, you will be able to access the website that allows you to submit an appeal after you accept your offer of admission and create a CalNet ID.

Requst for Confidential Information  This form is used to request copies of documents that contain confidential information.

Eligible Non-Citizen Statement 2018-19 Only use this form if you have been directed to do so by a UC Berkeley Financial Aid and Scholarships Office representative. If you are required to fill out this form, you need to submit it in person to our office.

Request for Confidential Information 2018-19 This form is used to request copies of documents that contain confidential information.
Supplemental Foreign Income Information 2018-19 Only use this form if you have been directed to do so by a UC Berkeley Financial Aid and Scholarships Office representative. It is used to report foreign income.
U.S. Citizenship Confirmation 2018-19

This form is used when proof of U.S. citizenship is required. Only use this form if you have been directed to do so by a UC Berkeley Financial Aid and Scholarships Office representative. If you are required to fill out this form, you need to submit it in person to our office.

Application for Additional Food Assistance  If you lack access to food in the short-term and are waiting for your CalFresh benefits, or you are not eligible for CalFresh benefits, applying to our food assistance program may be an option for you. 
Housing Security Deposit Application  The Housing Security Deposit Award Fund was established to empower student success in the face of economic injustice. This self-generating award is funded by student fees and was created through a popularly supported student fee referendum. Students are now helping enrolled students who need assistance in paying security deposits to obtain safe, secure, and stable housing
Summer Childcare Expense Appeal  You may qualify for funding in summer to help with childcare expenses. Complete this form and submit supporting documentation to apply.
